Patient Forms

To make your first visit more convenient, you can save time by filling out our new patient form in advance. Simply download, print, and complete them at your convenience before your appointment.

Dental Insurance

We file all major insurance company plans. Contact our office insurance coordinator to verify in-network participation with your plan. Va Beach Smile does not participate in Health Management Organizations (HMO); however, our practice will be happy to file your insurance claims. Our practice accepts cash, checks, and credit cards. We also offer a flexible outside financial partner with payment options.

Dental Financing

At Va Beach Smile, we provide multiple payment choices to accommodate our patients. We gladly accept major credit cards, cash, and checks. For those without insurance or in need of extensive dental treatments, we collaborate with CareCredit, a third-party financing option that offers affordable payment plans, some of which come with minimal to no interest.
